Requesting Your Health Care Records

You have the right to request a copy of your health care records. The best way to request your records is through your secure client portal: https://claridadcounseling.sessionshealth.com

Client Portal (preferred): Log in to your client portal and send a secure message requesting your records.

Email (alternate): If you're unable to access your client portal, you may email your request to info@claridadcounselingtx.com. Because email is not a fully secure method of transmitting personal health information, records requested this way will still be delivered through your client portal whenever possible.

Please include your full name, date of birth, dates of service, and your preferred format for receiving the records. To protect your privacy, we may ask for additional identity verification before releasing records.

Requests will be processed in accordance with Texas law and the HIPAA Privacy Rule. If additional time is needed to fulfill your request, we will notify you in writing with the reason and expected completion date.
